Group Play Form

• Group Play: All dogs must complete a temperament evaluation. Town & Country Pet Care reserves the right to refuse or reject any dog that may be a hazard to itself, other dogs in our care, or staff members. This includes, but is not limited to, dogs who are aggressive, antisocial, toy or people possessive, anxious or physically ill. All dogs must be spayed or neutered to participate in full group play day care program.
• Vaccination: All dogs must have current vaccinations against Rabies, Bordetella, Distemper, Parvo, and the Bivalent Canine Influenza. Dogs will not be admitted into Town & Country Pet Care Group Play Daycare until we have proof of vaccinations.
• Food: We ask that owners bring their own food from home so that pets do not develop any gastrointestinal upset from diet changes.
• Service Fees: Payment is due when service is rendered. Town and Country Pet Care Center accepts personal check, cash, and major credit cards (Discover, Visa, and MasterCard).
• DAYCARE: Pick up Policy: All pets must be picked up before Town & Country Pet Care Doggie Daycare closes at 6:00pm. A late fee of $5.00 will be charged for every 15 minutes after closing a dog remains at our facility up to a total of $10.00. If not picked up by 6:30pm dog(s) will spend the night.
• Leashes: No retractable leashes. If your pet is brought in on a retractable leash we will use one of our leashes to bring you pet back to you.

This is a required form for all Town & Country Pet Care Center participants receiving services.

Insuring that your pet(s) remain safe and well cared for is our first responsibility and as such we take it very seriously. If your pet becomes ill or injured at our facility we will call you and/or your emergency contact.

In the event that a medical emergency arises while a pet is at our facility or participating in a service that we provide it is imperative that we are immediately able to get them medical treatment at the closest available facility. Town & Country Pet Care Center will notify the owner as soon as treatment has started. Our goal is to get your pet(s) the medical attention they need as quickly as possible, and any distraction may interfere with that process.

If my pet should escape, injure themselves, refuse food, soil themselves, become ill, or die while in the care of Town & Country Pet Care Center, I will hold Town & Country Pet Care Center, its owners, and their staff free of any responsibility and /or liability in the absence of negligence.

I understand that in the event of a medical emergency Town & Country Pet Care Center, at its sole discretion; will decide the attention of a licensed veterinarian is needed. I authorize Town & Country Pet Care Center to seek medical attention at the closest available veterinary facility. I further agree to take any and all financial responsibility agreed upon above for medical treatment provided in the face of illness, injuries, or emergencies. I give Town and Country Pet Care Center the permission to act as an agent on my behalf of myself and my pet(s) to receive care in the face of a medical emergency while attending services provided by Town & Country Pet Care Center.
